It runs north and south through the towns of Islip and Smithtown between NY 27A in the town seat of Islip and the east end of the NY 25 and NY 25A concurrency in Smithtown's Village of the Branch.

Route description
In the town of Islip, NY 111 runs along Islip Avenue from NY 27A to County Route 100 (CR 100, named Suffolk Avenue), then Joshua's Path from Suffolk Avenue to CR 17 (Wheeler Road). The intersection with Caleb's Path was reconstructed in the mid-to-late 1980s in order to prevent congestion & accidents with the nearby Suffolk Avenue intersection. Taking over Wheeler Road from CR 17, it then crosses under the Long Island Expressway (Interstate 495) and makes a sharp right after intersecting CR 6, crossing NY 454, then shortly becoming conncurrent with CR 76 (Town Line Road) in Hauppauge. In the town of Smithtown, NY 111 runs along Hauppauge Road crossing an intersection with NY 347 continuing northward until finally reaching Smithtown.
History
NY 111 was assigned as part of the 1930 renumbering of state highways in New York. It initially followed its current alignment from Village of the Branch south to Central Islip,[1] where it turned slightly southeast to follow Wheeler Road and Carleton Avenue south to Main Street (originally NY 27 and now NY 27A) in East Islip.[3] On September 13, 1966, NY 111 was realigned to follow Joshua's Path and Islip Avenue instead, moving the route's southern terminus to Islip. The former routing of NY 111 on Wheeler Road and Carleton Avenue was transferred to Suffolk County and redesignated as County Route 17.[4]
Sometime before 1956, Route 111 went through Suffolk CR 76 (Town Line Road), rather than briefly overlapping the road.[citation needed]
North of Mount Pleasant Road, an interchange for the extension of Northern State Parkway was supposed to have been built.[5][6]
