The route is signed as east–west; however, it runs more in a northeast–southwest direction. The western terminus of NY 184 is at an intersection with NY 58 in the community of Pope Mills within the town of Macomb. Its eastern terminus is at a junction with NY 812 in the village of Heuvelton located within the town of Oswegatchie.

Route description
NY 184 begins at an intersection with NY 58 east of Black Lake in the hamlet of Pope Mills within the town of Macomb. The route heads northeastward, paralleling Fish Creek into the town of De Peyster. About 2.5 miles (4.0 km) from the town line, Fish Creek turns southward toward Mud Lake; however, NY 184 continues northeast in the vicinity of a marshy area comprising part of the creek's drainage basin. The swamps end near the community of Kokomo Corners, which NY 184 passes through.
North of Kokomo Corners, the route follows a more pronounced northeasterly routing into the town of Oswegatchie. At the town line, NY 184 begins to run along the Oswegatchie River and follows the riverbank to the village of Heuvelton. In Heuvelton, NY 184 is known as Union Street and serves as a local residential street. The route ends near the village center at a junction with NY 812.
History
NY 184 was assigned ca. 1931 and has not been changed since.[1][2]
