It serves as a connector between NY 166 in the Otsego County town of Roseboom and NY 10 in the Schoharie County town of Seward. NY 165 is a two-lane highway its entire length.

Route description
NY 165 begins at an intersection with NY 166 in the hamlet of Roseboom within the town of the same name. It heads southward through Cherry Valley to the community of Pleasant Brook, located on a small stream with the same name. Here, the route turns to the east and follows Pleasant Brook through a mountainous, isolated area of Roseboom. It passes through the hamlet of South Valley before crossing from Otsego County to Schoharie County. Now in the town of Seward, the mountains give way slightly as the route proceeds through the communities of Dorloo and Seward. NY 165 comes to an end at a junction with NY 10 north of the hamlet of Janesville.
History
The entirety of NY 165 was assigned as part of the 1930 renumbering of state highways in New York. In Roseboom, NY 165 was originally routed on modern County Route 57.[1] It was moved onto its current alignment in the mid-1960s.[3][4]
