Plot synopsis

Clemente is paranoid. Jody's husband keeps sending him condolences cards, the latest one contains dead fish. He is afraid to go home and suspicious of strangers including med student Jane. Things only get worse when Bobby calls him up with threats. He tries to get Abby to write him a prescription for something to help him sleep but she tells him to go and see his regular doctor.

Weaver has finished her physiotherapy regime after her hip replacement and now doesn't need to walk with a crutch. For the first time she takes an unaided walk through the ER.

Sam is put on edge when her ex Steve rolls in. He has suspected appendicitis but Clemente suspects him of faking it. Sam treats him, trying to remain professional. Neela is the only one who insists on a full work-up and when Steve's condition worsens it appears she was right to be concerned. He is taken up to the OR.

Abby treats two victims of a car accident, twelve year old Ruthie and her grandmother. The two don't have any serious injuries but Abby becomes concerned when Ruthie reveals she was the one driving the car. She brings this up with the grandmother but finds the older woman confused and suffering from the effects of Dementia. Ruthie has nowhere else to go and pleads to stay with her grandmother, stating that she can take care of them both and showing great maturity for her age. Abby discusses the case with Luka and he tells her that children shouldn't be carers, likening the situation to Abby's own childhood with her bipolar mother. As they argue they see Ruthie running away from the hospital, causing Abby to doubt her judgement. When Ruthie returns though she has an armful of books on how to care for her grandmother. Abby agrees to fight to let her stay with her grandmother, noting though that she will have to have home help care and regular visits from social services.

Morris has started as a drugs rep and can’t wait to flash around a new suit. The rest of the ER staff are unimpressed but a patient takes to him immediately. Dr. Albright and Morris disagree about treatment of the patient, he thinks there is no need for surgery and proves it by sticking a needle into the patient as Albright tries to drag the bed towards the elevators. What comes out is cloudy fluid, not blood - Morris was right. Afterwards Albright confronts Morris on his behaviour in the drugs lock-up. The two argue for a moment before Albright grabs Morris for a passionate kiss.

Pratt is on his way to the Sudan. He is packing his cases when he notices some earrings on his table. He leaves these at the hospital for Olivia. When she comes around to the ER Frank gives them to her. She admits they’re pretty - but they don’t belong to her. On the aeroplane Pratt starts to flirt with an attractive woman called Vatima. He tries to impress her with his profession and destination. She quizzes him on the problems in Darfur and Pratt's enthusiasm ebbs. When the plane lands Pratt holds up the line while his bags are searched. A paperback causes particular concern and Debbie has to step in to move the line along. As Pratt and Debbie travel to the IDP camp Pratt shows a flippant attitude to the situation that irritates Debbie. The janjaweed ride up on horses and force their car to stop. The driver is given a broken nose but Pratt and Debbie aren't harmed when they say they're doctors. Their medical supplies are taken, their car is made unusable and their shoes are stolen. Their only option now is to walk to the camp.

When they arrive Pratt has to bandage his bleeding feet. When he takes a walk through the medical tents he gets into trouble straight away by walking in on a rape exam. He meets with Dr. Dakarai and reunites with Carter who asks for news of County. Pratt tells him about Luka and Abby expecting a baby which Carter contemplates for a moment then declares this good news. He starts to talk about Clemente but they are interrupted by a disturbance in the camp. A member of the janjaweed has been captured and is surrounded by the camp’s men who are beating him with sticks. Pratt tries to rush in and stop them but is knocked to the floor. As he watches a teenage boy smashes the man's head in with a rock. Later Pratt drinks some alcohol he smuggled in a mouthwash bottle. This wasn’t what he was expecting at all.
