State Route 154 is an east–west running state route located entirely in Columbiana County in northeast Ohio. It runs though generally rural areas of east central Columbiana County from downtown Lisbon, Ohio at the intersection of U.S. Route 30 and state routes 45, 164, and 517 to the Pennsylvania state line near Negley, Ohio where it is redesignated as Pennsylvania Route 251. The route continues toward Chippewa, Pennsylvania.

• 1924 - Originally routed from Lisbon to East Palestine along current route 517 from Lisbon to East Fairfield, route 558 from Fairfield to 2 miles (3.2 km) east of Fairfield, and route 46 from 2 miles (3.2 km) east of Fairfield to East Palestine.^[2]
• 1926 - Rerouted from Lisbon to Pennsylvania state line along current routing.^[2]
• 1926 - Lisbon to East Palestine decertified.^[2]
