Niagara County Route 5 is an East-West route in the Towns of Lewiston, Cambira, and Lockport. Upper Mountain Road begins at its west end within the Town of Lewiston at the Tuscarora Indian Reservation Line, this since there are no county routes within Indian Reservations. Route 5 intersects New York State Route 429 (Townline Rd.) in the hamlet of Pekin, on the town line of Lewiston and Cambria. As Route 5 continues east through Cambria and reaches the center, it intersects New York State Route 425 (Shawnee Rd.), which is split in two by Route 5, so NY 425 runs a very short distance concurrent with Route 5. Upper Mountain then comes to the Cambria-Lockport Town Line where it intersects New York State Route 93 (Lockport Junction Rd.), and travels another convert|1.9|mi|km in the Town of Lockport until coming to an end at an intersection with NY 31 (Saunders Settlement Rd.) and NY 93 (Lockport Bypass).

New York State Route 93 was formerly routed along Route 5/Upper Mountain Road between NY 31 and the the Cambria-Lockport town line (current intersection with NY 93), however during early 2007, NY 93 was re-routed down NY 31 and then down Junction Rd. (Former NY 270) to eliminate truck traffic on Upper Mountain Road while providing additional capacity, resulting in the truncation of NY 270 to its junction with Saunders Settlement Rd. in Pendleton.
