It begins in the town of Somers at NY 100 and ends at NY 121 west of the hamlet of Waccabuc. The road passes by the shopping center at Golden's Bridge.

Route description
NY 138 begins at NY 100 south of the hamlet of Somers along Goldens Bridge Road. The road heads south then east, crossing over the Muscoot Reservoir just north of historic Bridge L-158 to meet I-684 and NY 22 in Golden's Bridge, about 1.8 miles (2.9 km) from the western end. Access to I-684 is for the southbound lanes only and there is no exit from I-684. The intersection with NY 22 is also grade-separated. The Golden's Bridge Shopping Center can be found at this junction. Beyond Golden's Bridge, the road continues east for another 3.4 miles (5.5 km) as Waccabuc Road into the town of Lewisboro. The route ends at NY 121 west of the hamlet of Waccabuc.
History
NY 138 was established in the 1930 state highway renumbering and has not had any major changes since.
