Its western terminus is at State Route 528 on the southern boundary of Madison, and its eastern terminus is at State Route 193 nearly 7 miles (11 km) southeast of Jefferson.

- 1935 – Swapped alignments with State Route 84[2].
- 1938 – Extended to 3 miles (4.8 km) east of Painesville along previous and current unnumbered roads[2].
- 1970 – Truncated at 1-mile (1.6 km) south of Madison; from 3 miles (4.8 km) east of Painesville to 1-mile (1.6 km) south of Madison decertified[2].
