- Ablative brain surgery
Ablative surgery is the surgical
ablation by burning or freezing ofbrain tissue to treat neurological or psychological disorders. Thelesion s it causes are irreversible.Uses
Parkinson's disease
Surgical ablation has been used to treat
Parkinson's disease . In the 1990s, thepallidum was a common surgical target. Unilateral pallidotomy improvestremor anddyskinesia on one side of the body (opposite the side of the brain surgery), but bilateral pallidotomy was found to cause irreversible deterioration in speech andcognition . Thethalamus is another potential target for treating tremor; in some countries, so is thesubthalamic nucleus , although not in theUnited States due to its severe side effects. Generally, deep brain stimulation surgery (DBS) is considered preferable to ablation because it has the same effect and is adjustable and reversible.cite web |url= http://www.waparkinsons.org/edu_research/articles/Stereotactic_Surgery.html |title= Stereotactic Surgery |publisher= American Parkinson Disease Association |last= Samii|first= Ali |date= 2002 |accessdate= 2008-05-01 ] However, for patients for whom DBS is not feasible, ablation of the subthalamic nucleus has been shown to be safe and effective. Cluster headache s have been treated by ablation of thetrigeminal nerve , but have not been very effective. Other surgical treatments for cluster headaches are under investigation.cite web |format= PDF |title= Stereotactic and Functional Neurosurgery News |url= http://www.assfn.org/society/Fall07newsletter.pdf |publisher= American Association of Neurological Surgeons |date= Fall 2007 |accessdate= 2008-05-01 ]Psychiatric disorders
In the
People's Republic of China , surgical ablation was used to treat psychological and neurological disorders, particularlyschizophrenia , but also includingclinical depression ,obsessive-compulsive disorder , andepilepsy .cite news |last= Zamiska |first= Nicholas |title= In China, Brain Surgery Is Pushed on the Mentally Ill |publisher= "The Wall Street Journal " |date =November 2, 2007 |accessdate= 2008-05-01] The officialXinhua News Agency has since reported that China's Ministry of Health has banned the procedure for schizophrenia and severely restricted the practice for other conditions.cite news |last= Zamiska |first= Nicholas |title= China Bans Irreversible Brain Procedure |publisher= "The Wall Street Journal " |date= April 28, 2008 |accessdate= 2008-05-01]Methods
Laser s have been shown to be effective in ablation of both cerebral and cerebellar tissue. A laser called , for example, allows great precision in location and size of the lesion and causes little to no thermal damage to adjacent tissue.
