Plot

Kim goes into labor, initially exciting J.D. But J.D. becomes uncomfortable when Kim begins pressing J.D. for a talk about their relationship and how they feel about each other; J.D. decided in the previous episode that he had no longer loved her, and was staying in the relationship only for the baby's sake. After J.D. reveals his true feelings, Kim is shocked and outraged. She, in turn, decides to end the relationship and summons Elliot to help her through labour. J.D. is sad when the relationship ends, citing statistics that say that children with parents who are together are better off. However, Dr. Cox tells him that statistics aren't always right, and that J.D. is the only one who can determine whether or not he will be a good father. J.D. then reenters the room, braving Kim's abuse, in order to be with her and his newborn son, Sam. The episode ends with Kim holding Sam and commenting that, while their (romantic) relationship is over, it is also just beginning (as parents), before allowing J.D. to hold Sam.

Turk and Carla are conflicted over a video game Turk is obsessed with. While J.D. was previously helping him as a second player, he leaves when Kim goes into labor, so Turk teams up with Janitor to finish the game. Carla catches Turk, and reveals that she is also addicted to the game, which is why she wanted it out of the house. After Carla annoys the Janitor into leaving, they finish the game together. Turk is euphoric over having beaten it, but Carla informs him she's done it so many times.

Dr. Kelso is revealed to be living in the hospital after his wife kicked him out of the house six weeks prior. He spends the night on the Doctor's Lounge couch drinking scotch and talking on the phone with son Harrison, whom he is helping through a recent breakup.

Dr. Cox is facing a crisis himself, as his baby daughter Jennifer-Dylan has a spreading rash; He doesn't want to give her a shot, out of fear that she will associate him with pain. An extremely lazy Jordan cannot be bothered with it, and Dr. Cox is subjected to finding someone suitable to give his daughter a shot, believing the interns too incompetent. In the end, Keith gives Jennifer-Dylan the shot.

Continuity

Aloma Wright's new character, credited as Nurse Shirley.

This episode marks a returning appearance for three actors. Aloma Wright, last seen in "My Long Goodbye" when her character Laverne Roberts died, returns as a nurse present while Kim is in labor. Though Wright has a new hairstyle and pink scrubs, she speaks like Laverne did. J.D. notes she looks familiar, but does not make the connection.

Also making a return appearance is Colin Hay, who made his first appearance in "My Overkill". In this episode he sings Men At Work's hit single "Down Under" (also sung by J.D. in "My Porcelain God").

Dr. Donna (Debra Azar), who delivered Jack Cox in "My Karma", delivers Sam Dorian.

When Dr. Cox is asking for help with his daughter, Jennifer Dylan, Dr. Kelso is drinking from a mug labeled "Plomox"; Julie Keaton, played by Heather Locklear, was Plomox's representative in "My First Step" and "My Fruit Cups".

In addition, when Turk is playing the XBOX 360 one of the controllers is wireless while the other is a wired controller with the wire cut off.

Also while two players are playing the video game, only one player on the screen is ever seen.
