It is believed to be the longest state highway in the United States with a "900" number. NC 903 is a spur of NC 90, which was later superseded by US 64. The current NC 90 runs from Statesville to Edgemont in the western portion of North Carolina.

Route description
The highway actually begins as secondary state road VA 903, running from U.S. 1 in South Hill, Virginia to the northern terminus at the North Carolina state line near Lake Gaston. From there, NC 903 begins and proceeds south over Lake Gaston to Littleton where it junctions with US 158 and NC 4. NC 903 turns left and follows US 158 for a short ways and then breaks off at Thelma...heading southeast to Halifax, crossing NC 48 and I-95 (Exit #168 off the interstate) along the way. Upon reaching Halifax, NC 903 will merge with NC 125 before running into US 301. The two highways will turn right onto US 301 and proceed south towards Enfield, passing over NC 561 along the way. NC 125 and NC 903 will soon break off US 301 at Ruggles and continue southeast to Scotland Neck and meet with US 258. The two highways will take a right onto US 258 passing through downtown where NC 903 will turn left and head southeast towards Hamilton, crossing NC 11 and NC 42 en route.
After passing through Hamilton and concurring NC 125 once again, NC 903 takes a more southerly route as it travels to Robersonville, meeting NC 142 a quarter of the way down. NC 903 will cross over US 13 and US 64 before entering Robersonville and then US 64 Alternate passing through downtown. As NC 903 comes into Pitt County, the highway will take a right at Beargrass Road near Stokes and cross over NC 30...now heading southwest to Greenville. NC 903 will join US 13 and NC 11 north of the city, then proceed south into Greenville as it crosses many more highways including US 264, NC 33, NC 43 and US 264 Alternate. The concurrency will drop US 13 and pick up NC 43 along the way to Winterville, where NC 903 will turn right and head in a westerly direction towards Snow Hill.
NC 903 will pass NC 102 at Roundtree and cross NC 123 at Maury before meeting with US 13 and US 258 near Snow Hill where the highway will once again merge. US 258 and NC 903 will break off at NC 91 as US 13 stays straight. NC 903 will keep straight as US 258 turns left to bypass Snow Hill. Upon reaching NC 58, NC 903 will take a right and then an immediate left. NC 903 will continue south to La Grange and overpass US 70 on the far side of town, where the highway will make an immediate right as it heads towards Kenansville. On the way, it will cross NC 55 at Whitfield Crossroads and merge with NC 111 at Albertson. The two highways will continue south to NC 11 at Kornegay where the concurrencies will switch with NC 903. Upon reaching Keanansville, NC 903 will break off and bypass the town to the east where it will merge once again...this time with NC 24, crossing over NC 50 and back over NC 11. Before entering nearby Magnolia, NC 24 will jump on I-40 (Exit #373 off the interstate) while NC 903 continues straight into town to junction with US 117. Still heading southwest in the direction of Garland, NC 903 will cross over US 421 at Delway...nearing its southern terminus. The highway will turn left at Lisbon Bridge Road and end a short distance further at NC 411.
