Plot
Part 1
After Elliot reminds him that it is their seventh year anniversary of their first kiss, J.D. freaks out and shoplifts a present for her. After Leonard retrieves it, Elliot tells him that they don't need to sweat the small stuff because they know what to expect in their relationship. Later, at the cafeteria, everyone receives invitations for the Janitor's wedding in the Bahamas. Elliot is upset that he is getting married before she is. Everyone is upset that the announcements came on such short notice, but J.D. convinces them that they all need a break and should be there for Janitor. They all agree. Elliot decides to stop eating so she will look good in a bathing suit. Later that night, in bed, she tells J.D. that she loves him. He tells her he feels the same way, but she explains that he must say it with feeling. He tries to sneak sweet ways to say "I love you" on her when they reach the Bahamas. After being yelled at by the Janitor for actually showing up, J.D. notices that all of the couples in the Bahamas are arguing. He confidently tells Elliot that they can rest assured with each other's love. He thinks he has pulled off a smooth move but Elliot walks away, upset.
Meanwhile, Turk is upset that Carla has been paying more attention to their daughter Izzy than to him. She is hesitant to go to the Bahamas because of leaving Izzy alone for such a long time. On the islands, she is constantly on her phone with the babysitter. When Turk convinces her to meet for a special rendezvous in the ocean, she stands him up and talks to Izzy on the phone.
Jordan pesters Dr. Cox into coming to the Bahamas, but he informs her that he will be busy working there. She celebrates this idea. However, she gets fed up with him being on his laptop all the time when they are together on the beach. She eventually hurls his laptop into the ocean when he reveals that he finished his work on the plane and was just doing extra work to prove his point.
Part 2
Facing the arguments that started the previous day, the couples from Sacred Heart are not having the best vacation they would like in the Bahamas. The Janitor has problems of his own - he must now organize a wedding ceremony. He asks The Todd to find a Justice of the Peace at an emergency international Brain Trust meeting, with new member Ira. Later, Todd introduces Janitor to Van (played by a cameo appearance from the creator of Scrubs - Bill Lawrence), an old buddy who let Todd practice rectal exams on him in medical school. At the wedding ceremony Janitor requested that everybody stand while he and Lady sat. Van does a terrible job leading the wedding by listing pitfalls he and his wife encountered when they were married. In the end, Janitor tells Lady that he loves her because she accepts him for who he is, in all his weirdness.
J.D. acts as if it is Elliot's fault that she is upset, after promising him there would be no more drama. She tells him that there is drama in every relationship, and he will need to be romantic to her if he truly loves her. Turk tells the same to Carla, complaining that she is too busy being a mother to have sex with him. This angers Carla, who accuses him for acting like a child, yet furthering her role as a mother. Cox tells Jordan that he truly loves her, but Jordan is reluctant to show her feelings for him. After the wedding, Carla finds some time to set aside to spend romantically with Turk. At the bar, Jordan tells Perry that when Perry is around she enjoys life more, which makes him happy. On a pier, J.D. opens his heart to Elliot, telling her that he loves her more than he ever thought he could love anybody, and that he loves her more than he loves Turk. He then gives her Turk's tiki necklace. J.D. then calls Elliot his "dream girl", a reference to the pilot episode where J.D. also calls her his dream girl, the two kiss during a Bahamian sunset.
At the bar of the resort, Dr. Kelso drinks Bahama Mamas all day long, and even sleeps at the bar. He misses the wedding because he is drunk. Ted tries to not wear sunblock into the sun for mere seconds and gets severely burned. Back at Sacred Heart, Doug locks himself in a morgue drawer when he finds out that he wasn't invited to the Bahamas for the wedding.
