In 2007 Rhode Island Hospital physicians operated on the incorrect side of a patient's head on three separate patients during three separate brain surgeries. The incidents occurred in January, July and November. Critics blame failure to use "universal protocol," and the nurses failure to speak up to the surgeons for the errors.
