Detectives Benson and Stabler investigate allegations of child abuse when Christopher Ryan (Jae Head), is caught driving a runaway van. Christopher explains how his foster parents, Gwen and Noah Sibert (Julie Bowen and Luke Perry), bribe him in exchange for abusive medical testing. As Detective Benson continues to study the boy’s accusations, she is led back to a former unsolved rape case involving Caitlyn Ryan (special guest star Sara Gilbert). After reviewing both cases, the detectives uncover a connection between the two cases that is complicated, when they meet SVU'S new ADA Kim Greylek (Michaela McManus), who quickly butts heads with everyone. While visiting a bar, Munch talks to Fin about how he used to own a bar in Baltimore and also mentions several people from his past that have left him. The names mentioned included previous partners on SVU (Monique Jeffries and Brian Cassidy) as well as some from Homicide: Life on the Street (Meldrick Lewis and Stanley Bolander), the series from where Munch's character originates. The history behind the bar is also from that series.

Detectives Stabler and Benson investigate when Eric Byers (Marshall Allman) admits to the squad about his sexual advances towards his younger stepbrother. Eric admits to also looking at a website to try to curb his pedophilic urge. With no crime committed, the detectives decide to investigate the website for pedophiles leading them to the creator, Jake Berlin (Tom Noonan). When Stabler discovers his daughter Elizabeth's picture on the website he confronts Jake, beats him up and is suspended. When Eric goes missing his whole family is under investigation.

Detective Stabler is called to the scene of a breaking and entering at a home. The trespasser’s wallet leads him to his daughter, Kathleen (Allison Siko). Stabler is shocked to find his daughter is abusing drugs and behaving promiscuously. It is determined that Kathleen has bipolar disorder, but she refuses to admit it. Elliott must turn to his estranged mother Bernadette Stabler (special guest star Ellen Burstyn), as she is also bipolar to testify in court that bipolar runs in the family. To help Kathleen, Detective Benson must step in to save both of them before things get out of control.

Detectives Elliot Stabler and Olivia Benson investigate when a famous astronaut, Marga Janssen is found dead, and Detective Stabler's old mentor, Dick Finley (special guest star James Brolin), helps investigate the case. They first believe the rape is linked to a string of serial rapes, but they are soon led to look for a suspect in the group of people Marga worked with. An obsessed fan is questioned, but is released and gets into a gun battle with Dick. Stabler becomes suspicious of Dick after he keeps pointing them away from him, but it's revealed that Dick killed Marga because she was stealing his spotlight.

When a baby is found with advanced AIDS, Detectives Benson and Stabler are called in to investigate why someone would let a baby go untreated for HIV. They are lead to a highly unethical doctor, Gideon Hutton (guest star Martin Mull) an AIDS denialist who believes and teaches that HIV doesn't cause AIDS and offers alternative treatments to cure HIV. They search for more families that have been influenced by Gideon and are lead to a particular family whose younger daughter died of AIDS and the mom and son are called into court along with Gideon.

The investigation into the burning death of a homeless teenager leads Detectives Stabler and Munch to an elite high school where they meet high school students Max Matarazzo (Jesse McCartney) and senior Alec Bernardi (Philip Ettinger), who confesses to the crime. Alec admits to the murder because he thought his sister was raped by the man, but in truth his sister and her friends made a pregnancy pact and willingly had sex to become pregnant. However, the case takes a strange twist when the ringleader of the pregnancy pact apparently commits suicide, after being harassed online by parties unknown who the detectives believe to be the mother of one of the girls (Debi Mazar).

His marriage once again on the rocks, Detective Stabler goes undercover to expose an animal smuggling ring when a woman turns up dead mauled to death by a tiger and she has a rare species of bird in her purse. They are lead to Hip Hop artist, Gots Money (Big Boi) who helps Stabler infiltrate the smuggling ring, but when Detective Benson poses as a prostitute to help out her partner undercover, Detective Stabler finds himself wounded after his identity is uncovered, forcing Benson to take drastic action to avenge her partner.

A woman named Mia (Clea Duvall) claims to have been raped in an alley to hide being abused and raped by her husband (Nathaniel Marston). Benson gets her to admit the abuse, but Mia recants which ends in her murder at the hands of her husband. While solving this crime, Benson stumbles upon another case of spousal abuse and murder which involves Mia's older neighbor Linnie (Brenda Blethyn) who is revealed to have been involved in an abusive relationship, which ended when she killed her former husband, and escaped custody under the supervision of Judge Donnely (Judith Light), who was greatly humiliated as a result.

Benson and Fin investigate the murder of a pregnant marine who was raped by one of her fellow marines in Iraq, leading to the two getting a first hand look at how women in the military are treated. The investigation leads them to suspect a fellow marine killed her, but they face difficulties when Navy Commander Grant Marcus (Frank Whaley) threatens to shut down their investigation. During the proceedings, Benson's sexual assault during the previous season stirs up painful memories for her as the investigation takes several twists, with regards to the prime suspect and the real rapist and his wife, who was responsible for the murder.

Kelly Sun (Kelly Hu) is found wandering Riverside Park, beaten and raped, and has no memory of what happened. Detective Benson discovers Sun was on her way to Korea, but never boarded her flight and instead is found leaving the airport with a mystery man, Eric Lutz (Michael Trucco). Lutz becomes an immediate suspect, and with further probing, the detectives find rape-themed amateur pornographic videos on Lutz's computer. Detectives Stabler and Benson search for incriminating evidence against Lutz, but each of his victims from the videos suffers from memory loss from the attack due to the use of date rape-related drugs. Attempting to trace the other women in the videos, the detectives must re-inform each victim of her attack to get her testimony. Struggling to remind each victim of their rape, Detective Benson must face the fact that she still thinks like a victim, and get the testimony of one victim in particular, Laurel Andrews (Christy Pusz), to put Lutz away for good. Complicating matters, Judge Moredock is blackmailed based on sexually explicit pictures found on his private server.

Heather Hallander (Ellen Woglom) miraculously returns home after being reported missing over four years ago. Unrecognizable to her family, Heather tells Detectives Olivia Benson and Elliot Stabler of the cement cell where she spent the past four years as a sex slave and of her fortunate escape. Heather’s sister, Erica Hallander (Kate Baldwin), is relieved to see her long-lost sister after launching a website asking for search tips, but Nikki Hallander (guest star Natalia Payne), Heather’s other sister, seems annoyed with her return. In search of the mystery kidnapper, Benson and Stabler drive Heather around town to help stir up any memories of her abduction. When Heather’s answers don’t seem to match up, the detectives find revealing evidence that make them question Heather’s kidnapping and true identity.

When the body of a fourteen year-old girl is found floating in the Hudson River, Detectives Benson and Stabler think she was smuggled into the country by sex traffickers. Detective Tutuola discovers some new evidence that could change the entire case.

When young girl Rosie Rinaldi (guest star Daisy Tahan) is kidnapped, the girl's mother, Liz Ranaldi (guest star Michelle Ray Smith) immediately points Detectives Benson and Stabler in the direction of her ex-husband, Geno Parnell (Ron Eldard), a recently paroled convict. When Parnell's alibi checks out, a fight erupts between Parnell and Detectives Stabler and Tutuola, which in turn unveils some knowledge on Parnell's ex-wife. Parnell admits Liz's father is Frank Hagar (guest star Dabney Coleman), a master thief with a lot of enemies. The detectives pay a visit to Hagar, but are surprised to find he has a severe case of Alzheimer's disease. With the help of Dr. Huang, the detectives are able to put together a few clues to solve the case, but must pair up with Parnell to find Rosie.

When a man is found badly beaten in a strip club parking lot, Detectives Benson and Stabler are at a loss for evidence as a rainstorm washed away any clues. A fake fingernail embedded in the victim's back leads the detectives to believe that the attacker is female. The victim, Mark Van Kuren (guest star Frank Grillo), awakes in the hospital with no memory of what happened, but brings the attention to his ex-wife Ellen (guest star Wendy Makkena) and their 13-year-old transgender daughter Hailey (guest star Bridger Zadina). It turns out that Hailey hated her father for not accepting her as a girl, which makes her the prime suspect, but the investigation soon leads to her guidance counselor (guest star Aisha Hinds) instead. Daniela Sea guest stars as a transgender man. Alex Kingston guest stars as defense attorney Miranda Pond.

Pediatrician Gilbert Keppler (Lawrence Arancio) is found guilty of sexually assaulting four of his male patients, so Detectives Stabler and Benson decide to go directly to Keppler for questioning, only to find him murdered in his apartment. Fin tells them that they are waiting for Greyleck to show up to the crime scene. To the detectives' surprise, Greyleck doesn't show up. She has resigned because the Obama administration called her back to Washington, D.C. Her new replacement is SVU's former ADA Alexandra Cabot (Stephanie March), who has been assigned to take over Greyleck's caseload by Jack McCoy nearly three years after her release from the witness protection program and subsequent return to the DA's office. Stabler and Benson's investigation leads to Jeff Lynwood, the developmentally challenged son of a wealthy man (John Gallagher, Jr.), who killed Keppler because he called him stupid. As the trial goes on, the detectives discover that Jeffrey suffers from lead poisoning caused by Chinese-made products. Final appearance of Kim Greylek, though Michaela McManus would continue to be listed in the opening credits through the rest of the season.

When Detectives Benson and Stabler arrive on the scene of a single murder, they discover two more dead bodies, those of Tisa Flores and Alejandro Reyes, in the apartment next door. They investigate Tisa’s past and find that she was working at a strip club owned by a former Rockette, Birdie Sulloway (Carol Burnett). Birdie and her nephew Chet (guest star Matthew Lillard) try to assist with the investigation, but when the prime suspect for the three murders, Birdie's abusive husband suddenly dies, the detectives suspect Birdie may be a black widow.

A young girl, Miriam Burdett (Julyza Commodore), is found in an alley, with her throat cut and evidence of repeated rape and physical abuse. When asked to write who attacked her, because of her inability to speak, she draws a picture of the devil. When Detective Munch investigates, detectives meet Father Theo Burdett (guest star Robert Wisdom), Miriam's adoptive father, who rescued her and her friend Elijah Okello (Gbenga Akinnagbe) from the Lord's Resistance Army - one of whom may be her attacker. Wisdom and Akinnagbe co-starred together on the HBO series The Wire.

After an up-and-coming artist is brutally murdered in her apartment, Detectives Tutuola and Stabler link her death to another unsolved murder case. As they pursue the case further, they realize that these two murders have striking similarities to a string of murders by a mysterious serial killer that Detective Victor Moran (Delroy Lindo) has been following for months. Moran fights to keep his control over the case as Detectives Fin and Stabler struggle to find the killer.

When Ruth Walker reports the disappearance of her two-year-old granddaughter, Sierra, the detectives then suspect the girl's mother, Ashlee (Hilary Duff) of killing her daughter. The detectives however find out Sierra actually died of measles because another mother refused to vaccinate her child. Partly inspired by the death of Caylee Anthony.

Kim Garnet (guest star Carly Schroeder) is teased by her classmates about something she did and ends up tripping and falling down the stairs. The Special Victims Unit is brought in after a doctor's examination discovers that she has been a victim of physical abuse. There are two suspects: boyfriend Steve Walker (guest star Alexander Nifong) and a friend from school named Ethan Morse (guest star Ezra Miller) who has a bad temper, and one of them beats Kim up to influence her testimony. She will not disclose who it is in court, and Judge Hilda Marsden (guest star Swoosie Kurtz) therefore declares her in contempt and she is jailed. Kathleen Stabler (guest star Allison Siko) is brought in by Benson to give Kim advice, and she admits that Steve Walker thought she was having an affair with Ethan, so he sent the sexts in response. At trial, both sides agree to drop the charges, but Marsden disagrees and sentences Kim to one year in a facility in Ohio for sex offenders. After an investigation into Marsden's record, Olivia and Miranda Pond are denied documents, then arrested on contempt while confronting Marsden. The prosecutor, initially reluctant, sides with them and alerts Cragen who frees them. After bail and gaining info from a court officer involved in the scheme, it is revealed that she has been accepting bribes as well as turning small-time offenses into felonies. A sting is set up with Stabler giving Marsden a bribe, Ethan (who is into acting) taking on the role of the defendant, and Dr. Huang as the defense lawyer. The operation is a success, Marsden is caught and taken into custody, and Kim is released with her record cleared. This marks the first time Dr. Huang makes an on-screen arrest as an FBI Agent.

When Pamela Galliano (Sprague Grayden) seeks to have a restraining order reinstated against her ex-boyfriend, Tyler Brunson (Jon Patrick Walker), the assigned judge has something else on his mind. For peace of mind, Judge Koehler (Alan Dale) asks Detective Stabler to question a convicted serial killer about the whereabouts of his son's body, which adds a new twist to the case.

A tourist is found dead in Central Park with the word “guilty” written across her forehead, Detectives Benson and Stabler recover photos from the victim’s camera. With the help of crime scene unit technician Dale Stuckey, they discover that former artist Peter Harrison (guest star Nick Stahl) was at the park during the crime. After arresting Harrison for questioning, the detectives leave Detectives Munch and Tutuola to keep Harrison under close surveillance. But when Harrison suddenly escapes, Detective Munch must turn to his ex-wife Gwen (Carol Kane) for help. Carol Kane previously played the part of Gwen Munch in an sixth season episode of Homicide: Life on the Street.
