Ján Kozák (born 17 April 1954) is a retired Slovakian footballer.
During his club career he played for Lokomotiva Košice and Dukla Praha. He earned 55 caps and scored 9 goals for the Czechoslovakia national football team from 1976 to 1984, and participated in UEFA Euro 1980 and the 1982 FIFA World Cup.
