Since the formation of the Premier League in 1992, only four teams have won the title - Manchester United, Blackburn Rovers, Arsenal and Chelsea. With ten wins to their name, it is unsurprising that Manchester United players hold the top positions for being in title-winning sides. A player needs to appear in at least a quarter of his side's matches (currently 10) in a title-winning season to qualify for a medal. * Kevin Gallacher was a part of Blackburn's title-winning squad in 1994-95 but only featured in 1 game due to a broken leg
* David Batty was part of Blackburn's title-winning squad in 1994-95, but only played in 5 matches due to a broken foot. Batty was offered a medal under the "special dispensation" rule, but he refused it as he felt his contribution was minimal.
* Quinton Fortune was part of Manchester United's squad in their title-winning seasons of 1999-2000, 2000-01 and 2002-03, but did not play enough games to receive a medal in each of those seasons. Fortune was awarded special dispensation for season 02-03 and awarded a Premier League winners medal despite playing only 9 games.(See "Quinton Fortune)
* Gael Clichy is the youngest recipient of a Premier League medal.
