State Route 121 is a primary state highway in the U.S. state of Virginia. It connects an interchange at Fort Chiswell, where U.S. Route 52 leaves the Interstate 77/Interstate 81/U.S. Route 11 freeway, with the intersection of State Route 1004 (Ayers Street) and State Route 1007 (Horseshoe Drive) in Max Meadows. SR 1004 continues north several blocks to State Route 610 (Wythe County, Virginia).

SR 121 was numbered in the 1933 renumbering as an extension of U.S. Route 121 (now US 52 south of Fort Chiswell). From 1931, when it was added to the state highway system, until 1933, SR 121 was part of the longer State Route 15, which also continued south on present US 52. [CTB minutes|07-1931-01, page 70]
