Plot
Change doesn't come easy for the gang when J.D. considers moving closer to his son. Meanwhile, Turk gets comfortable as chief of surgery, while Ted and Gooch reach a new level in their relationship.
J.D. tells the rest of Sacred Heart that he will take a position at a different hospital to be closer to his son. This is met with mixed feelings as Dr Cox seems ecstatic at the fact that J.D. is leaving while Turk lashes out by making a decision about a patient without consulting J.D., which he first promised. Meanwhile the Janitor is concerned that J.D.'s leaving is going to trigger other people to drastically change their lives something that in turn might change his life. He even goes so far as to forbid Ted to move in with his girlfriend, something that Ted ignores.
Dr Cox is also verbally abusing the intern Jo for making a mistake with one of the patients and when being asked about why he was being so mean Cox indicates that he is really hurt that J.D. is leaving (although he does not confirm it when directly asked).
As it ends up, The Janitor accepts Ted's relationship moving forward as Turk admits to J.D. that he was, in fact, acting out. Turk knows that J.D.'s new job, his own new job, their children and the distance to J.D.'s new apartment makes it almost impossible for the two to maintain the same relationship they had.
The episode ends with J.D. leaving the hospital for the day, noticing a quiet tension between himself and Dr. Cox.
