Fat hand syndrome

Fat hand syndrome describes the upper limb changes noted following excessive exposure to ultraviolet radiation (sunburn) or trauma.

Pathophysiology

Being a localised acute inflammatory response, the four signs of Celsus are displayed [Rocha e Silva M. "A brief survey of the history of inflammation." 1978. Agents Actions 1994;43:86–90.] [Benaroyo L. "How do we define inflammation?". Schweiz Rundsch Med Prax 1994;83:1343–7.] . Marked pitting oedema (tumor) affects hand, usually sparing the wrist and digits due to the spacious dorsal compartment. The cardinal signs of calor, dolor and rubor are optimal at 12-24 hours.

Further causative factors are currently being researched, namely the combined exposure to sunlight, alcohol and seawater (An Daingean triad ).

Management

Although a self-terminating condition, duration can be reduced by anti-histamine therapy, limb elevation and avoidance of sunlight at peak times.
