The character Coffin Joe (Marins) is an evil, amoral undertaker whose singular obsession is to find the "perfect woman", one of a "superior" quality (primarily that of being able to endure torture tests), so that she may bear the child Coffin Joe feels will carry on his lineage. He has a general contempt for all people, and disdains and mocks all religion and philanthropy. He has a strong sense of entitlement to achieve his goals, and will exploit, maim and kill those who get in his way.

Released from a prison mental ward after 40 years, Coffin Joe renews his bloody quest for "continuation of the blood", the crime for which he was imprisoned in 1967. He returns the streets of São Paulo, seeking the "perfect" woman who is qualified to bear his "superior" child. He kidnaps and imprisons the most beautiful women in the town and puts them through sadistic tortures and tests as a measure of their superiority. Throughout these events, Coffin Joe is repeatedly haunted by the ghostly visions of his previous crimes, including Terezenhia and Antônio from "At Midnight I'll Take Your Soul".

Plot

Upon arriving at São Paulo, Coffin Joe finds shelter in an underground hideaway underneath a shanty town. The hideout is populated by a cult of psychotics under the leadership of Bruno (Rui Rezende). The cult has been anticipating Joe's arrival.

Coffin Joe sets out immediately on his quest for the perfect woman. With the aid of the cult, he seduces and imprisons Hilda, who is inflicted with punishments that include being administered drugs, mutilation, and cannibalism. He later kidnaps the young Elena, who he possesses under the crucified body of her blind aunt.

Being increasingly persecuted by the ghosts of his past crimes, Coffin Joe becomes more and more deranged. He attempts to cope with the visions by rationalizing that they are figments of his imagination. These vision finally result in Coffin Joe finding himself visiting a vision of Hell, a gruesome, intestine-like maze. There he is confronted by the Mystifier, who reveals to him disturbing and surrealistic horrors. Despite this terrifying experience, Joe continues to victimize helpless, beautiful women on his quest.

As in his quests decades before, Joe leaves behind a bloody trail of evidence of his crimes, and the local police try to Joe down and kill him, which necessitates his escape into an amusement park, pursued by old enemies, his victims' ghosts, and police. It is here that Coffin Joe meets his fate.
