The road runs from Farmington north to Conway.
The southern terminus of NH 153 is at New Hampshire Route 11 in Farmington. The highway winds north through Sanbornville and Wakefield. South of Effingham the road bends east, skirting the eastern shore of Province Lake, and crosses the border into Maine in the area of Newfield.[2] North of the lake, the road turns westward and then travels due north, with the road lying directly on the Maine/New Hampshire border for 1.2 miles (2 km).
As the road continues northward, it passes through Effingham Falls and Eaton. The northern terminus of NH 153 is at New Hampshire Route 16 and New Hampshire Route 113 in Conway. In Conway, NH 153 is named Pleasant Street.
