Alison Forman (born 17 March 1969 in Maitland, New South Wales) is a retired female football (soccer) midfielder from Australia. She captained The Matildas at the 2000 Summer Olympics in Sydney, Australia, where the national team ended up in seventh place.

Forman is one of the few Matildas with experience playing as a professional overseas. She played with Danish club Fortuna Hjorring for seven years, from 1992 to 1999. Along the way she collected honours, including three Danish Championship gold medals. She was named the league's player of the year in 1996-97.

Despite her success in Denmark, Forman returned home in 1999 to concentrate on the team's Olympic campaign. Forman is one of 20 players on full-time residential scholarships with the Australian Institute of Sport in Canberra.

Forman has been a part of both Australia's World Cup campaigns, in Sweden in 1995 and the United States 1999. She was vice-captain of the Matildas in the US. She played for Northern New South Wales Pride in the Australian League, having made her debut in 1999. She was one of the twelve players who appeared in the Matildas' nude calendar for 2000.
