Italy national football team from 1989 to 1990, and was included in the1990 FIFA World Cup squad when Italy finished third on home soil. Carnevale was a regular starter for the Italians in the run up to the tournament in friendlies and appeared in Italy's first two games.After being replaced by Toto Schillaci in Italy's opening game with Austria, Carnevale watched on as Schillaci scored 2 minutes later. Carnevale started the next game against USA but after he again failed toscore he was once more replaced by Schillaci. It turned out to be his last game for Italy.
Following the world cup Carnevale joined Roma from Napoli but after scoring 4 goals in his first 5 games he was then suspended for a year for drug use.
