It usually presents itself as a non-localised, non-pitting
edema and thickening of theskin which usually occurs on theanterior aspect of the lower legs spreading to thedorsum of the feet. In advanced cases, this may extend to the upper trunk (torso), upperextremities ,face ,neck , andears .The
lesions are known toresolve very slowly. It occasionally occurs in non-thyrotoxicGrave's disease ,autoimmune thyroiditis , andstasis dermatitis . The serum contains circulating factors which stimulatefibroblasts to increasesynthesis ofglycosaminoglycans .Diagnosis
A
biopsy of the affected skin revealsmucin in the mid- to lower-dermis . There is no increase infibroblasts . Over time, secondaryhyperkeratosis may occur, which may becomeverruciform . Many of these patients may also have co-existingstasis dermatitis . Elastic stains will reveal a reduction in elastic tissueReferences
