Plot
It is J.D., Turk and Elliot's last day as interns; soon they will become residents. At that point, they realize they have become jaded after working at the hospital for a year, and they decide to put in an extra effort to treat an elderly patient, Mr. Bober, who does not have the insurance to pay for a surgery he desperately needs. They enlist the help of Dr. Cox, Dr. Wen, and Jordan.
Meanwhile, Sacred Heart needs a new residency director, and Dr. Kelso tells Dr. Cox that he decided to recommend him for the job.
Also, the Janitor wishes a stunned J.D. well in his career as a resident. Moments later, however, he meets a new intern, after finding the front door to be inoperable; when the new intern suggests there is a paper clip in the door, the Janitor accuses him of putting it there, replaying his first encounter with J.D.
After Dr. Cox chides Jordan for her predictability, she ends the episode by dropping some bombshells: she tells them all about her ex-husband's feelings for Carla, Turk's secret awareness of those feelings, Elliot's feelings for J.D., Kelso's lies about the new job for Dr. Cox and, to J.D.'s horror, informs Cox that she and J.D. had sex when they first met. Everyone storms off, angry and embarrassed, leaving J.D. sitting alone.
Continuity
- The beginning of this episode mirrors the beginning of the first episode, "My First Day."
- The patient Mr. Bober reappears in the episode "My Brother, Where Art Thou?." However, by that point he has fairly serious dementia and can only say the word "pickles."
Featured music
(The DVD version replaces "Away" by Leroy with "Only One" by Jeremy Kay.)
