MeshID = D020820Hemiballismus is a
neurological sign that presents as amovement disorder , characterised by unilateral wild, largeamplitude flinging movements of the arm and leg, normally causing falls and preventing postural maintenance.Causes
It is caused by a
lesion orinfarction in the contralateralsubthalamic nucleus or its connections, usually in patients with a history ofhypertension ordiabetes , or following TBmeningitis . The subthalamic nucleus normally regulates theglobus pallidus by exciting the GPi, which in turn normally inhibits theventral anterior nucleus ,ventral lateral nucleus andlateral dorsal nucleus of thethalamus . The reduced discharge in both causes disinhibition of the thalamus and consequent involuntary stimulation of the motor cortex. Altered dopaminergic feedback mechanisms may also be involved.cite journal |author=Klawans H, Moses H, Nausieda P, Bergen D, Weiner W |title=Treatment and prognosis of hemiballismus |journal=N. Engl. J. Med. |volume=295 |issue=24 |pages=1348–50 |year=1976 |pmid=980081 |url=http://content.nejm.org/cgi/content/abstract/295/24/1348 |format=abstract]Treatment
Chemical treatment is via
dopamine blockade, or via surgical lesions to the globus pallidus, lenticular fasciculus or VL nucleus of thalamus.The prognosis is rather favourable both vitally and functionally ]References
