Form 3310.4

A Report of Multiple Sale or Other Disposition of Pistols and Revolvers or Form 3310.4 is a United States Government form that a firearm dealer must fill out when an unlicensed person puchases or acquires two or more pistols or revolvers at one time or during five consecutive days. This form is sent by the dealer to the Bureau of Alcohol, Tobacco, Firearms, and Explosives and to a designated State authority. [ATF Form 3310.4, Instructions]
