The development was authorized for OKB-586 of M.Yangel on 20 March 1958 (simultaneously with R-15), then transferred to SKB-385 of Makeev on 17 March 1959. It was assigned theNATO reporting name SS-N-5 Serb and carried theGRAU index 4K55 [ [http://www.aviation.ru/GRAU/#4K55 GRAU ] ] . The submarine missile complex was named D-4 for project 629B submarines.The D-4 launch complex with R-21 missile has entered service by a Decision of Ministry Soviet of the USSR of 15 May 1963. Retired in 1989. There were 21 submarines with D-4 complex aboard, 63 missiles on combat duty, 228 launches (193 successful, 19 failed due to hardware bugs, 11 failed due to personnel errors and faults of supplementory systems).
Fuel for the missile was designated "AK-27I/TG-02", which consisted of two liquids, a mixture of 73% HN03
