"The Most Sadistic" is a song by American rapper Necro, featuring his brother, Ill Bill, which was released in 1999. It was the first and only single from his debut album, "I Need Drugs", released the following year.

The song was officially remixed once since its initial release, in 2003 for "Rare Demos and Freestyles Volume 1".
