The name Pathirappally was derived from an ancient tharavad name called "Pathirappallil" which formed part of the "Mattathil " major tharavad (moola kudumbam) in the same location. Both the family houses are centuries old and formed part of the typical Kerala matrilineal type of landlord families which possessed acres of land in and around Pathirappally. It was the last Karanavar (family head) Mattathil Parameshwara Kurup (born 1895 and died 1964) who lived in the "Pathirappallil" Tharavad (nalukettu) who started a village Market in the location in the late 1940s when there was no pucca road even (presently there is NH47 passing through). He brought about qualitative changes in the local social infrastructure. Thus the market place name was derived from the tharavad name 'Pathirappallil' and known as Pathirappally. The SCMV UP School at Poomkavu near Pathirappally was also founded by him and later surrendered to the government.
