medicine , a toxidrome (portmanteau oftoxic andsyndrome ) is a syndrome caused by a dangerous level oftoxin s in the body. Commonsymptom s includedizziness ,disorientation ,nausea ,vomiting , andoscillopsia . A toxidrome may indicate amedical emergency requiring treatment at apoison control center . Aside frompoison ing, a systemicinfection may also lead to a toxidrome. The most common toxidromes are classified as:
*
Anticholinergic
*Cholinergic
* Hallucinogenic
*Opiate
*Sedative /hypnotic
*Sympathomimetic Anticholinergic toxidrome
The symptoms of an anticholinergic toxidrome include blurred vision,
choreoathetosis ,coma , decreasedbowel sounds, delirium , dryskin ,fever , flushing, hallucination s,ileus ,memory loss ,mydriasis (dilatedpupils ),myoclonus ,psychosis ,seizure s, andurinary retention . Complications includehypertension ,hyperthermia , andtachycardia . Substances that may cause this toxidrome includeantihistamine s,atropine ,benztropine ,datura ,tricyclic antidepressant s, andscopolamine .Due to the characteristic appearance and behavior of patients with this toxidrome, they are
colloquial ly described as "Hot as a Hare, Dry as a Bone, Red as a Beet, Mad as a Hatter, Blind as a Bat".Cholinergic toxidrome
The symptoms of a cholinergic toxidrome include
bronchorrhea ,confusion ,defecation ,diaphoresis ,diarrhea ,emesis ,lacrimation ,miosis ,muscle fasciculation s,salivation ,seizure s,urination , and weakness. Complications includebradycardia ,hypothermia , andtachypnea . Substances that may cause this toxidrome includecarbamate s,mushroom s, andorganophosphate s.A mnemonic device to remember the signs/symptoms of organophosphate toxicity is SLUDGE- Salivation, Lacrimation, Urination, Diaphoresis, Gastrointestinal distress, and Emesis.
Hallucinogenic toxidrome
The symptoms of a hallucinogenic toxidrome include
disorientation ,hallucination s, hyperactive bowel sounds, panic , andseizure s. Complications includehypertension ,tachycardia , andtachypnea . Substances that may cause this toxidrome includeamphetamine s,cocaine , andphencyclidine .Opiate toxidrome
The symptoms of an opiate toxidrome include altered mental states,
miosis , shock, and unresponsiveness. Complications includebradycardia ,hypotension ,hypothermia ,shallow respiration s, and a slow respiratory rate. Substances that may cause this toxidrome includedextromethorphan ,opiate s, andpropoxyphene .edative/hypnotic toxidrome
The symptoms of a sedative/hypnotic toxidrome include
ataxia , blurred vision, coma ,confusion ,delirium , deterioration ofcentral nervous system functions, diplopia ,dysesthesias ,hallucination s,nystagmus ,paresthesias ,sedation , slurred speech, andstupor .Apnea is a potential complication. Substances that may cause this toxidrome includeanticonvulsant s,barbiturate s,benzodiazepine s, andethanol .ympathomimetic toxidrome
The symptoms of a sympathomimetic toxidrome include
anxiety ,delusion s,diaphoresis ,hyperreflexia ,mydriasis ,paranoia ,piloerection , andseizure s. Complications includebradycardia ,hypertension , andtachycardia . Substances that may cause this toxidrome includealbuterol ,amphetamine s,cocaine ,ephedrine (Ma Huang ),methamphetamine ,phenylpropanolamine (PPA's), andpseudoephedrine .External links
