description=The medal is a bronze disc in which an embossed triangle was superimposed in the center of the disc and the Philippine sun is placed over the triangle. The scroll in bronze is placed at the lower portion of the disc with its end supporting the tendrils of the wreath that is made of sampaguita flowers and buds arching toward the center top of the medal.
The sun signifies the Philippines and its eight sunbursts represent the provinces that revolted against Spain; the triangle and three stars represents the three major islands of the Philippines; and the wreath stands for meritorious and outstanding achievement.
The ribbon has a white background with green stripe in the center.
