"Be Careful what you wish for" is the 12th book in R.L. Stine's Goosebumps series.

Directly from R. L. Stine

Samantha Byrd is a klutz. An accident waiting to happen. She's the laughingstock of the girls' basketball team and that mean, rotten Judith Bellwood is making her life miserable off the court. One afternoon, Sam gets off the bus from school, she runs into Clarissa, a creepy lady who grants her three wishes. Only she wasn't careful when she made those wishes, and now that they're coming true, her life is turning into a living nightmare! In the end, Judith gets three wishes and wishes that Samantha Byrd would just fly away. This is when Samantha turns into a bird, stretches her wings and flies off.
