- Transition from Prison to Community Initiative
The Transition from Prison to Community Initiative was proposed and developed by the National Institute for Corrections as a comprehensive model for the reintegration of convicted felons back into the community.
Guidelines for implementation
*Planning for release begins at intake
*Assessment and classification instruments must be validated for prison population
*Offenders’ time must be guided by a program plan
*Plan must be linked to risk and needs
*Plan must have buy-in, collaboration and accountability for all stakeholders: Inmate, Family, Community Supervision, Other State and Private Agencies, and Faith and Community based organizations.
*Program and services must be related
*Based on evidence-based practices
*Performance management throughout system
*Information must be shared seamlessly
