She was born Susan Maria Smith in Weeksville, Brooklyn, New York and grew up on her father's pig farm at the corner of Fulton Street and Buffalo Avenue. In 1867, when she was 20 years old, she began her studies at the New York Medical College and Hospital for Women in New York City. The college taught homeopathy. She graduated on March 23, 1870, and after her graduation, she opened an office in her Brooklyn home and later a second one in Manhattan. Her first husband and father of her two children was William G. McKinney, a reverend. In 1893 she married Theophilus Gould Steward, a reverend. Theo Steward was the chaplain of the 25th United States Colored Infantry, known as Buffalo Soldiers. Susan gave lectures to the public about health, nutrition and medicine. She often spoke about women’s rights and the progress of African American women. In 1881, she founded the Memorial Hospital for Women and Children. She cared for senior citizens at the Brooklyn Home for Aged Colored People. She died at the age of 71 on March 7, 1918. The Susan Smith McKinney Junior High School, in Brooklyn, New York, was named in her honor on September 25, 1974. Later, the Susan Smith McKinney Steward Medical Society was founded by a group of African American women doctors.

She was buried in Green-Wood Cemetery in Brooklyn.
