Plot
The new Chief of Medicine, Dr. Maddox, is keeping a strict eye on the doctors — so much so that J.D. complains to Elliot the hospital is becoming like a prison and that sometimes it's necessary to bend the rules to help their patients. Dr. Cox thinks he's managed to sneak a patient into a clinical trial, but Maddox finds out about it and sends the patient home. J.D. and Elliot keep prescribing medication for a patient even though they know it's actually for his daughter, who has no insurance. After Maddox proudly tells them she saw through the deceit and sent both father and daughter home, Elliot, J.D. and Dr. Cox approach Dr. Kelso to ask his help in persuading the board to fire Maddox. In return, Kelso forces Dr. Cox to admit that he misses him.
Intern Katie is stepping on the other interns to get ahead and Carla tries to "set her straight". Katie argues that the hospital is so competitive that no one looks out for each other. Carla warns her that eventually everyone would hate her and that she needs to surround herself with friends in order to survive.
