Certified Case Manager

Certified Case Managers are certified by The Commission for Case Manager Certification and participate in a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services available to a person in need of health services. The case manager is fundamentally a voice of experience on behalf of a person receiving care or in need of receiving care. Speaking to the patient, evaluating the care they are receiving and ensuring the care is appropriate and making sure any concerns the person has are voiced by either suggesting questions to ask their health care providers or by directly involving themselves in the care and suggesting needs management to the clinicians.

Case managers also speak to insurance companies and health care equipment companies on behalf of the person to ensure every aspect of their care is covered all while attempting to minimize the strain on the person they are representing and providing good outcomes for their healthcare.

Certified Case Managers can be Registered Nurses, Registered Respiratory Therapists, or any other licensed clinician not requiring observation by another licensed clinician.
